HOW IT WORKS
Super Simple
Step 1: Collect as many shoes as you can
Step 2: Fill out the online form so we can keep track of the shoes
Step 3: Bring them into the Sequoia Lobby and put them in the black donation bin
Step 4: The school makes money and helps people in need

DONATION FORM
To be eligible for prizes and so we can keep track of the shoes donated, you must fill out this form before dropping the shoes off. Please note, room teachers are not responsible for keeping track of shoes--you are!
What you will need to fill out the form:
1) Your Name
2) Your Child's Name
3) Grade
4) Room Number
5) How many pairs you will be dropping off
6) If you can, take a picture of the shoes like the one below. This is not mandatory

Once this is filled out, simply drop off the shoes in the lobby--there is a black bin you can place them in. Please make sure pairs are put in the same bag, loose shoes will not be counted.

That's it...eazy peazy!
The drop off station is in the school lobby. Please just place your shoes in the black bin provided, we will sort and bag the shoes.
FAQ
We will add to this every time a question comes in
How many shoes can we collect? There are no limits to how many we can collect and how much money we can raise.
What types of shoes can we donate? Athletic Shoes and Cleats–any condition. The school will receive more money for gently worn shoes that can be worn again. Shoes with holes or that are damaged will be recycled and we still get paid for them. See the guidelines here: Compensation and Donation Guidelines
What are the prizes?
Grand Prize: top one (2) shoe collectors in the entire school will get to pick the prize of their choice
All other prizes will be a raffle based for everyone in the school, regardless if you donated!

We have received donations from the Warriors, A's and others.
What if I forget to fill out the form? This is completely on the honor system, if you turn in the shoes and don't fill out the form then please go back and do it and upload another picture. This whole drive is to benefit the school, so the more shoes the better!
